After submitting this form you will receive an email invoice for the required non-refundable deposit of $25.00. Upon payment of the deposit you will receive a second email invoice for the remaining balance of $225.00 due no later than January 28, 2021. *Please note registration is not final until paid in full. Any refunds are solely at the discretion of the CPM Office. No refunds will be paid after the start of the session. *
